Organization Assessment Questions

With the version 3.3.1 release, MDRAP includes three new questions added to the MDISS Questionnaire to gather information on the time and resources used to complete each assessment and to document the version number of the device that is being assessed.  These questions do not appear in the MDS2 or the SF (Short Form) questionnaire types. The following questions were added:

1. Total time (in minutes, 10 - 9999) to complete the assessment? Please include all persons assisting with completing assessment (e.g. If two staff spend 35 minutes completing survey, total time = 70).

3. Version of device assessed (may be found on boot screen, startup screen, service mode, or application screen). 

There are multiple ways to access the questions, as described in sections a. - d., below.

1. Login to MDRAP and navigate to the Assessments page by clicking on the Go to Assessments link in the middle of the screen or clicking on the Assessments tab in the blue navigation bar across the top of the screen. 

a. Access the questions using the icon: On the Assessments page, click on the Nabla symbol/upside down triangle icon (highlighted in yellow in the screenshot below) that is available for all MDISS questionnaires in your organization's Assessments list. 

(1) A pop-up box will appear displaying the three added questions.

(2) Enter the answers to the questions for the selected assessment and click on the SAVE button to save your answers in the MDISS questionnaire.

b. Access the questions by selecting an MDISS Questionnaire for a device from your organization's list of assessments: as shown in the screenshot below the MDISS Assessment for the Aleris PCU 8000 Series is being selected.

(1) Navigate to the bottom of the list of Control Categories and click on Other MDS2 Questions.

(2)  The first question in the series of Other MDS2 Questions will be displayed. Navigate through all of the MDS2 Questions by clicking on the NEXT button until you get to the end of the questions. In the screenshot below there are a total of 8 questions you will need to click through.

(3) After clicking on the NEXT button for the last question in the series, the Organization Assessment Questions pop up box will appear on the screen.

(4) Enter the answers to the questions for the selected assessment and click on the SAVE button to save your answers to the MDISS questionnaire.

c. Access the questions via the Assessment Results page: Navigate to the Assessments page and select View Results icon.

(1) The Assessment Results page will be displayed. Select the Nabla symbol/upside down triangle icon at the top of the page as shown in the screenshot below.

(2) The Organization Assessment Questions pop up box will appear on the screen.

(3) Enter the answers to the questions for the selected assessment and click on the SAVE button to save your answers in the MDISS questionnaire.

d. Access the questions via the Assessment Questions page: Navigate to the Assessments page and select an MDISS Questionnaire for a device from your organization's list of assessments (as shown in the screenshot below the MDISS Assessment for the Aleris PCU 8000 Series is being selected).

(1) Click on the MDISS Assessment link (e.g., MDISS Assessment for Alaris PCU 8000 Series, and the Assessment Questions page will be displayed with the Organization Assessment Questions Nabla symbol/upside down triangle icon. The MDISS Assessment will display the Nabla symbol/upside down triangle icon on the top right of every page throughout the Assessment questionnaire.

(2) Click on the Organization Assessment Questions Nabla symbol/upside down triangle icon and the Organization Assessment Questions pop up box will be displayed on the screen.

(3) Enter the answers to the questions for the selected assessment and click on the SAVE button to save your answers to the MDISS questionnaire.
