View Assessment Report

To view an Assessment Report in your organization's inventory:

1. Log in to MDRAP ACCOUNT LOGIN.

2. On the MDRAP Home Page, click on the Go to Assessments link in the middle of the screen or click on the Assessments tab in the navigation bar across the top of the screen.

3. The list of assessments for your organization will be displayed. 

4. To view an Assessment Report, click on the magnifying glass icon to the right of the Assessment selected for viewing.  

5. The Assessment Report will open in a new screen, which can be viewed as an HTML page for all assessments that have not been submitted. The Assessment Report includes a header detailing: the Device Name, Model Number, and Manufacturer; Type of Assessment; Last Updated Date; Completed by (Organization);  Organization's Device Location, Care Delivery Area, Serial #, and Asset Tag #. NOTE: Even if the assessment is shared, organizational info is not shared, i.e. completed by, locations, CDAs, serial# & asset tags are not shared outside the organization.

6. The PDF Assessment Report icon is displayed for all Submitted assessment reports. 

7. Clicking on the PDF Assessment Report icon opens the Assessment Report in a new window. The Assessment Report includes a header detailing: the Device Name, Model Number, and Manufacturer; Type of Assessment Completed; Submitted Date; Completed by (Organization); and Organization's Device Location, Care Delivery Area, Serial #, and Asset Tag #. NOTE: Even if the assessment is shared, organizational info is not shared, i.e. completed by, locations, CDAs, serial# & asset tags are not shared outside the organization.

8. In addition to the View Assessment Report and PDF Assessment Report icons, users can also view MDS2 assessments using the MDS2 Form icon as highlighted in the screenshot below.

9. Clicking on the MDS2 Form icon opens the completed MDS2 Form in a new window.
